Ultimate Care NY is seeking a Clinical Coordinator who is passionate about delivering exceptional patient care and ensuring seamless coordination of healthcare services. This role plays a vital part in maintaining clinical compliance and supporting efficient communication between patients, nursing staff, and managed care plans. Under the direction of the Clinical RN Manager, the Clinical Coordinator will promote efficient client care, coordinate essential healthcare services, and uphold agency clinical compliance.

Compensation : $21-$25 per hour, based on relevant experience.

Location: 1000 Gates Avenue, Brooklyn, NY, 11221

Schedule: Monday-Friday 9:00AM-5:00PM Onsite

You will:

  • Coordinate patient care by interacting professionally and compassionately with clients and nursing staff.
  • Facilitate clear communication with managed care plans to support continuity of care.
  • Process referrals to medical services, ensuring timely access to necessary care.
  • Assist patients in obtaining medical equipment as directed by nursing staff.
  • Prepare, mail, and manage patient documentation, including medical records and required forms, while maintaining confidentiality and HIPAA compliance.
  • Maintain organized and up-to-date patient records in accordance with healthcare regulations.
  • Provide administrative support on departmental projects and other tasks assigned by leadership.
  • Act as a liaison between field nursing staff, the Clinical Department, and other internal teams to support smooth operations.

You will have:

  • Minimum of 1–2 years of Home Care experience required; experience in a clinical setting is a plus.
  • At least two years of administrative experience in a healthcare setting required.
  • Experience with HHAeXchange preferred.
  • Proficiency in navigating electronic health record (EHR) systems.
  • Strong working knowledge of Microsoft Office Suite, including Excel, Word, and Outlook.
  • Bilingual proficiency in English and Spanish preferred.
  • Strong interpersonal and communication skills to foster collaboration across departments.
  • Detail-oriented with a strong focus on accuracy in addressing client needs, resolving issues, and creating a positive client experience.
  • High level of customer service, patience, and problem-solving skills.
  • Ability to prioritize tasks and manage responsibilities effectively to meet deadlines.
