What You’ll Do:

• Provide one-on-one physical therapy to adult patients in their homes.
• Perform evaluations, develop treatment plans, and deliver skilled interventions.
• Adjust care plans based on patient progress and functional goals.
• Coordinate directly with patients to manage visit scheduling.
• Complete documentation using our user-friendly EMR system.
• Maintain compliance with Medicare guidelines and company standards.

What We’re Looking For:

• Licensed PT in New York State.
• Experience with adult or geriatric populations (home care experience a plus, not required).
• Strong communication skills and ability to build patient rapport.
• Comfortable working independently in a home-based setting.
• Basic familiarity with EMR systems (training provided).

About the Role:

This is an in-home position , not a single-site clinic role. Patients are assigned based on your preferred geographic coverage area across NYC and Long Island.
