The Charles B. Wang Community Health Center, an award-winning federally qualified community health center, is seeking a Nurse Practitioner for full-time coverage. We are looking for team-oriented individuals interested in delivering high quality primary care in a community health setting. Our collegial outpatient-only practice offers strong ancillary support, including nursing support, referral coordinators, health educators, on-site mental health services, and social workers. Responsibilities- Involve with direct patient physical assessment, education, and health planning
- Maintain continuity of care by regular patient follow-up
- Assist in the coordination of services with other disciplines and patients' insurance plans
- Advocate for patient in arrangement of medically necessary services
- Communicate with community resources to ascertain adequate assistance for patients in need
- Refer patients to specialty services when necessary
- Consult with collaborating physician and other members of the health care team to establish plan of care
- Participate in quality assurance process to assure optimal care delivery
- Provide comprehensive case management services that include emphasis on health promotion and disease prevention
- Monitor patient outcome in accordance to the practice protocols
- Collaborate with staff nurses and nursing administrators with assessment and clinical decision in patient care planning
- Act as a resource person for the nursing staff
- Conduct education in service on practice issues
- Perform other duties as assigned
