POSITION OVERVIEW
POSITION SUMMARY
this job offer will be responsible for developing a professional trusting relationship with clients and community providers in order to ensure coordination of services and positive results. Will be responsible to provide a full range of services to clients and their families, including care/case management, advocacy, information, referral, visits to the clients home. The part time Care Manager will assist the client and his/her family to understand, accept, and follow medical recommendations and will provide services which are planned to restore the client to as optimal a level as possible within the clients capacity. The Care Manager is responsible to provide an initial data assessment for the establishment of a person-centered care plan that coordinates and integrates a comprehensive array of a beneficiarys clinical and non-clinical services, in coordination with an interdisciplinary team. In the part-time managery function, the employee will supervise Level 1 and Level II care managers.ESSENTIAL tasks AND RESPONSIBILITIES include the following. Other tasks may be assigned.
»Coordinate referrals as assigned, and respond to referrals within a reasonable time to facilitate management through health intervention.
»Interview the patient to assess decision making, coping expertise, and barriers to managing their healthcare needs.
»Assist patients and families by providing support and education regarding availcapability of identified services.
»Supervise other Care Managers
»Monitor service delivery, provide weekly supervision, conduct team meetings, train others
»Represent Parsons at monthly care management agency meetings
»Maintain access to MAPP for assignments and billing
»Assign cases located on acuity
»The care manager must be able to work closely with others and work with patients in both routine and stressful situations related to the patients medical, physical and behavioral health condition.
»Must be able to incorporate/utilize the translation resources available to ensure that language does not become a barrier of care.
»Will adhere to policies and procedures related to:
»Mandatory education and Annual health assessment
»Ensure that Continuing education via Health Stream is completed annually
»Will prepare and maintain a variety of records and reports of client progress
managerY RESPONSIBILITIES
Directly supervises non-exempt employees. Supervises and evaluates the work of individuals; has responsibility for subordinate performance and output.QUALIFICATIONS

EDUCATION and/or practice
Requires a Bachelors degree in Social Work or a related field and at least 2 years practice, or a Masters degree in a related field with one year of practice, including practice providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism, or substance abuse or care management services.
Requires the capability to drive, must possess a valid drivers license and automobile that is insured in conformity with New York State Requirements
Must be able to work well with individuals of various backgrounds, age, ethnicities, life positions, and socio-economic statuses
