Program: Children and Family Services
Responsibilities: Working independently will conduct assessments; create service and discharge plans; provide direct clinical services via telehealth, in the home, at the office and/or in the community as needed; provide goal-directed supports and solution focused interventions to help children achieve goals; provide stabilization sessions
after a crisis episode; provide referrals to and coordinate with other service
providing agencies; document all direct and collateral services and interventions in
a timely manner; maintain case records; participate in meetings as directed; and
performs other related tasks as assigned.
General Requirements: LCSW, LMSW or LMHC required. Master’s degree in mental health field,
designated by the NYS Office of Mental Health, e.g., licensed mental health
practitioner. practice working with children (mental health, educational or childwelfare
settings) desirable. Two years post-graduate practice working with
individuals recovering from serious mental illness preferred. Excellent verbal and written
communication expertise. Deep knowledge of recovery process and principles of
person-centered planning. Comfort going into the community, meeting children
and their families. Knowledge of community resources and mental health services.
Proficiency with Electronic Health Records preferred. practice working with a
diverse population. Valid driver’s license and capability to drive program vehicles.
