Job Summary:

The Program Manager for Diabetes Services is a key member of the endocrinology team, primarily responsible for providing individualized and group-based diabetes self-management education and counseling to outpatients across the WMCHealth Network. This role focuses on empowering patients to achieve optimal diabetes control and overall health through education, medical nutrition therapy, and behavioral support. In addition to direct patient care, the Program Manager helps coordinate diabetes education initiatives, maintains compliance with regulatory standards, and supports quality improvement efforts within the outpatient endocrinology setting.

Responsibilities:

• Provides comprehensive outpatient diabetes self-management education (DSME) and medical nutrition therapy (MNT) to individuals and groups under physician referral, in accordance with CMS and professional practice guidelines.
• Collaborates with endocrinologists and other providers to develop personalized, evidence-based care plans that include nutrition, insulin management, glucose monitoring, and lifestyle modifications.
• Educates patients and families on key diabetes topics, including carbohydrate counting, medication management, sick day protocols, and prevention of complications.
• Utilizes motivational interviewing and behavior change strategies to support patient adherence and self-efficacy.
• Monitors patient progress, adjusts goals as needed, and documents care within the electronic medical record system in compliance with regulatory standards.
• Offers flexible scheduling to meet patient access needs.
• Supports patient outreach and referral management to ensure timely access to diabetes education services.
• Serves as a liaison between the diabetes education program and other departments including Primary Care, Nursing, Nutrition, etc.
• Participates in endocrinology team meetings, care conferences, and strategic planning discussions related to outpatient diabetes management.
• Contributes to the development of educational materials and standardized workflows in support of network-wide diabetes care coordination.

Experience:

Minimum of three (3) years of experience in outpatient diabetes education or chronic disease management. Experience working within an endocrinology or ambulatory care team preferred.Familiarity with CMS DSME/MNT requirements and electronic medical record documentation preferred.

Education:

Bachelor’s Degree in Nursing, Nutrition, or a related clinical field required. Master’s Degree preferred.

Licenses / Certifications:

Current NYS license in applicable field (e.g., RN, RD). Certified Diabetes Care and Education Specialist (CDCES) required or eligible within 12 months of hire.

Other:

Strong communication, counseling, and patient engagement skills. Ability to work collaboratively with multidisciplinary teams. Proficient in data entry, patient tracking, and program coordination. Comfortable supporting quality improvement and regulatory readiness efforts Special Requirements: Primarily outpatient setting, with occasional travel to affiliated endocrinology clinics or satellite locations. Requires flexibility in scheduling to accommodate patient needs.Light physical duties such as walking, sitting, and lifting educational materials.
