Job Title: Pharmacist - Contract to Hire / Direct Hire


Location:** Syracuse, NY


Start Date:** ASAP


End Date:** 13-weeks


Shift:** Mix of days/evenings, rotating weekends


**About the Role:**
As a Pharmacist, you'll be at the forefront of operations, ensuring seamless medication management for our patients. Your responsibilities will include:


- Serving as the Pharmacist of Record for the integrity of the clinical prescription review process (PV1)
- Reviewing, entering, and clarifying prescriptions from nurses and physicians (PV1)
- Preparing medications with precision and care, detecting any therapeutic incompatibilities (PV1)
- Dispensing medications through compounding, packaging, and labeling
- Monitoring drug therapies and providing interventions as needed


**Key Responsibilities:**
- Efficiently handle various forms of orders: faxed, verbal, electronic, and paper
- Conduct initial and final review of orders (PV1 & PV2)
- Collaborate with physicians to obtain prescriptions (PV1)
- Maintain EKits and Automated Dispensing Machines
- Oversee controlled substances and backorders
- Compound IV/TPN and non-sterile compounds
- Ensure accuracy in NDC changes and pre-packed medications (PV2)
- Supervise pharmacists and order entry staff
- Manage OOP/LOA requests and triage documents (PV1)
- Perform clinical reviews and report medication errors (PV1)
- Handle billing procedures and pharmacy operations


**Requirements:**
- Knowledge of medications, interactions, and dosage
- Experience in long-term care preferred, but not required
- Familiarity with Frameworks and Docutrack information systems beneficial
- Current pharmacist licensure in NY
- Software proficiency: Frameworks and DocuTrack
