Job Summary

Under general supervision, the Case Worker provides social services to clients and patients, including individuals receiving public assistance, nutritional assistance, and medical benefits. The role supports adults who are receiving or may require institutional care or protective services.

The Case Worker is responsible for identifying, developing, and implementing individualized social service plans, determining eligibility for services, and supporting clients through counseling, coordination, and case management activities. Investigatory responsibilities may also be required as part of case oversight.

Key Responsibilities

• Assess client and patient needs and develop appropriate social service plans

• Determine eligibility for public assistance and related programs

• Coordinate placement services, including nursing home placements

• Perform ongoing case management and follow-up activities

• Conduct counseling and investigatory work as required

• Initiate and manage referrals using electronic care coordination systems (CarePort and EPIC preferred)

• Collaborate with interdisciplinary teams to ensure continuity and quality of care

Required Qualifications

• Minimum of 3 years of relevant case work experience

• Direct experience with case management and nursing home placement coordination

• Experience initiating and managing referrals

• Strong written, verbal, and documentation skills

Education

• Bachelor’s degree from an accredited college or university

Preferred Skills

• Experience with CarePort and EPIC systems

• Prior experience in a hospital or healthcare setting
