Registered Nurse (RN) Homecare Supervisor – Per Diem Home Care

Attentive Care is seeking an experienced and compassionate Per Diem Registered Nurse Supervisor to support our growing home care services throughout the Capital Region. This role offers flexibility while providing the opportunity to make a direct impact on the quality of care delivered to clients in their homes.
As an RN Supervisor, you will oversee clinical services, conduct supervisory visits, provide guidance to caregivers, and ensure compliance with all applicable home care regulations and agency standards.
Compensation
Per Visit Pay Structure
- Supervisory Visits: Competitive per-visit rate
- Reassessments: Competitive per-visit rate
- Initial Client Assessments/Admissions: Enhanced rate for evenings and weekends
Mileage reimbursement provided at the current IRS rate.
Service Area
Albany, Schenectady, Rensselaer, Saratoga, and surrounding Capital Region communities.
Schedule
- Per diem
- Evening and weekend availability
- Flexible scheduling based on client and agency needs
Responsibilities
- Conduct in-home supervisory visits to monitor the quality of care provided by aides and caregivers.
- Complete nursing assessments, reassessments, and admissions for home care clients.
- Develop, review, and update individualized plans of care in collaboration with physicians and interdisciplinary team members.
- Provide clinical instruction and support to home health aides, personal care aides, and field staff.
- Evaluate caregiver performance and identify training opportunities.
- Monitor client status and communicate significant changes to physicians, caregivers, and agency leadership.
- Maintain accurate, timely, and compliant clinical documentation.
- Participate in staff education, orientation, and quality assurance activities.
- Ensure services are delivered in accordance with New York State regulations and agency policies.
Qualifications
- Current New York State Registered Nurse (RN) license in good standing.
- Current CPR certification.
- Minimum of one (1) year of experience as a licensed Registered Nurse (RN).
- Minimum of one (1) year of home care experience preferred.
- Prior supervisory, management, or leadership experience preferred.
- Strong assessment, communication, and documentation skills.
- Valid New York State driver's license, reliable transportation, current vehicle registration, and automobile insurance.
