Community Habilitation Worker

Job Summary
The Community Habilitation staff member plays a vital role in providing support and assistance to individuals with developmental disabilities, autism, and other special needs within their home and community environments. This position requires a compassionate individual who is skilled in communication and caregiving, dedicated to enhancing the quality of life for those they serve.

Duties:

• Provide direct care and support to individual with developmental disabilities and autism in their home and community settings.
• Assist the participant with daily living activities, including personal hygiene, meal preparation, and medication management.
• Implement individualized care plans tailored to each participant’s unique needs and goals.
• Communicate effectively with the participant, family, and other professionals involved in the care process.
• Drive clients to appointments, community activities, and social events as required (Driving optional).
• Monitor client behavior and implement and follow their pre-developed staff action plan to promote positive outcomes.
• Maintain accurate records of client progress and report any changes or concerns to the appropriate personnel.

Requirements:

• Strong communication skills, both verbal and written.
• Experience in home community care, special education, or childcare preferred.
• Knowledge of working with individuals on the autism spectrum or those with developmental disabilities.
• Ability to drive and possess a valid driver’s license.
• Compassionate demeanor with a commitment to providing high-quality caregiving.
• Strong organizational skills and attention to detail.
• Ability to work independently as well as part of a team.

Pay: $25.00 - $33.00 per hour
