We seek a Transition Specialist to conduct and monitor the implementation of transition services on a student IEP, such as training, education, and, where appropriate, independent living skills.

In this role, you will:

• Transport students to employment placements or job experiences. Physical aspects may include but are not limited to lifting and transferring of any student within the school setting.
• Coordinate services and supports for students, working with educators, families, and community agencies.
• Administer and interpret career and vocational assessments.
• Help match students' skills and interests with post-school outcomes.
• Identify community or adult services that can help students achieve their goals.
• Establish networks of support for families and students.

• High School Diploma or Equivalent
