Responsibilities
- Provide direct and indirect skilled nursing care for residents under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and the Interdisciplinary Team.
- Contribute to development of an age-appropriate, culturally relevant plan of care and implement nursing interventions to achieve resident outcomes with resident/family involvement.
- Monitor resident outcomes, evaluate effectiveness, and revise the care plan as needed; refer to appropriate services to address all aspects of care.
- Maintain current nursing knowledge and competency; participate in continuing education and in-service programs; mentor and orient staff as needed.
- Collaborate with physicians and other healthcare professionals to ensure coordinated, safe, and high-quality care for adult and geriatric residents.

Requirements
- Active NY Licensed Practical Nurse license
- Current Basic Life Support (BLS) certification through American Heart Association or RQI
- Minimum 1 year of experience as an LPN in a Long-Term Care setting
- Ability to care for adult and geriatric residents
- Ability to work rotating day/evening shifts; orientation can start on a Tuesday or Thursday
- Ability to cover holidays and weekends as required
