A Home Health Registered Nurse is needed in Rockland County, NY.

Salary and Benefits

• Salary range: $100,000 – $115,000 annually
• 25 PTO days in year one
• 6 paid holidays

Responsibilities

• Setting: Home Health
• Patient volume: 6–7 per day
• Productivity: 33 points/week
• Admission – 2 pts
• Recert/Resumption – 1.25 pts
• Routine Visit – 1 pt
• Infusion Admission – 2.5 pts
• Infusion Revisit – 1.5 pts
• Schedule: Mon–Fri, 9am–5pm
• Weekends: 2 weekend days/month (fee-for-service)
• No on-call duties
• EMR: Homecare Homebase (HCHB)
• Documentation: OASIS preferred

Requirements

• Active RN license in NY
• Associate or Bachelor’s degree in Nursing
• 1+ year clinical nursing experience preferred
• OASIS experience strongly preferred
• Home health experience a plus
• Reliable transportation required
