Transitional Care Assistant / Patient Care Coordinator
Now Hiring: Transitional Care Assistant / Patient Care Coordinator
Temp-to-Perm | Fully Remote | $27–$28/hr
Must live within driving distance for quarterly in-person meetings
Bilingual Spanish strongly preferred
We are seeking a compassionate Patient Care Coordinator to support patients after hospital discharge and ensure seamless continuity of care. If you have strong customer service experience in healthcare and a passion for helping patients access the care they need, you’ll thrive in this role.
What You’ll Do:
- Coordinate post-discharge services and follow-up care
- Schedule doctor appointments and arrange transportation
- Monitor patient progress and collaborate with interdisciplinary care teams
- Document all interactions in EMR systems
- Build relationships with physicians, nurses, case managers, social workers, and care providers
- Maintain timelines, accuracy, and strong follow-through
What We’re Looking For:
- 2+ years of customer service experience in healthcare (hospital, long-term care, home care, or medical office)
- Experience with outreach/enrollment and strong patient engagement skills
- Excellent organization, follow-through, and time management
- Proficiency in computer systems; Microsoft Office is a plus
- Bilingual Spanish highly preferred
