Were Looking For

A compassionate dedicated RN Case Manager who makes patients feel at homewhether theyre in their own home an assisted living facility or a skilled nursing home. If youre a hospice or home health Jedi with a valid GA RN license drivers license and at least one year of experience we want to meet you.

Your Role Should You Choose to Accept It

• Be a steady source of comfort and support to patients and families

• Help families understand disease progression self-care and nutrition

• Collaborate with the interdisciplinary hospice team to create and follow patient care plans

• Manage symptoms like a boss and advocate for patient comfort

• Maintain timely and accurate clinical documentation
