Sun River Health is currently seeking a full-time MAT Licensed Practical Nurse Clinical Coordinator to join our team in Beacon, NY.
The MAT Licensed Practical Nurse Clinical Coordinator is a part of multidisciplinary team who supports the specialty and primary care nurses and MAT program operations. Tasks include rendering nursing care to patients, within assigned unit of the center or authorized community site, in support of medical care as directed by the medical staff and pursuant to the objectives and policies of the center. Participate in the coordination of patient services, maintains MAT rosters, participates in case conferences, and supports the region's opioid overdose avoidance program. Performs job tasks in compliance with systems and job specific standards.
Responsibilities:

»Coordinate services with the patient, providers, and community referral resources.

»Attend continuing education and training to support professional development and patient care.

»Act as a support to nurse champions at each site.

»Assist with panel monitoring at all sites maintaining accurate up-to-date program spreadsheets.

»Outreach to patients for scheduling, rescheduling, and maintaining patient engagement.

»Provide training and support to nurses supporting MAT waivered providers.

»Provide Harm Reduction education and training to staff and patients on MAT, Fentanyl Test Strips, Naloxone, and stigma.

»Interviews and maintains patient's medical record to include: nursing observation; health history to include past and present use of medication; vital signs; vision/hearing tests; medications and treatments given; Reactions to tests i.e. PPD measurements; laboratory test results, utilizes patient identifiers. (PEAS, PhQ, Pain, Smoke Status, etc.) CDSS.

»Perform in-house labs, collect specimens for out-side labs (ex. Urine drug screens, etc.) perform quality control in conformity with organizational protocols.

»Prepares equipment and assists physician during treatments and examinations of patients.

»Complete and update all assigned tasks & logs. (i.e. correctly recording shipments and vaccines received in appropriate areas, signing out meds & treatments in log).

»Initiates follow-up as necessary (as directed by provider) i.e. encounters, labs, close non-billables, etc.

»Provides Health education training with patients as indicated by physician, diagnosis and/or inquiries by patient.

»Maintains and collects program data for reports, grants, and participates in quality improvement initiatives.

»Complete Prior Authorizations for specialized medication and services.

»Attend case conferences and document case updates in patient record.

»Support patients with retention and program engagement and document accordingly within patient chart.

»Coordinate the distribution and completion of MAT intakes and initial program engagement.
Education/practice:

»High School Diploma/GED Required, Bachelor's preferred

»LPN Diploma Required

»Driver's License required

»CPR/BLS required from American Red Cross or American Heart Association in class only

»NYS valid LPN License

» 1 year minimum practice; 2-4 years preferred
Annual income : $60,000.00 - $65,000.00 / Year
