Title: LPN – Dialysis

Location: Rochester, New York

Schedule: Day/Evening Shifts | 40 Hours Weekly

Shift Timing: Flexible scheduling between 5:30 AM – 10:30 PM

Position Overview:

"Navitas Healthcare, LLC" is seeking an experienced LPN – Dialysis to provide high-quality care for chronic outpatient and acute dialysis patients. The ideal candidate will have strong dialysis experience, excellent patient care skills, and the ability to float between dialysis locations as needed.

Primary Responsibilities:

• Provide direct patient care in outpatient and acute dialysis settings

• Perform dialysis treatments and monitor patient response

• Cannulate AV fistulas and AV grafts

• Administer blood and blood products as required

• Monitor dialysis equipment and respond to alarms appropriately

• Perform blood glucose monitoring and patient assessments

• Maintain infection control and patient safety standards

• Recognize and respond to dialysis-related complications and emergencies

• Document treatments and patient progress accurately

• Float to other dialysis locations based on facility needs

Required Qualifications:

• Active LPN License required

• BLS Certification required

• Minimum 1 year of Dialysis experience required

• Chronic Outpatient Dialysis experience required

• Acute Dialysis experience required

• Strong knowledge of dialysis equipment and procedures

Required Clinical Skills:

• AV Fistula and AV Graft Cannulation

• Tunneled and Non-Tunneled Catheter Care

• Fresenius Dialysis Equipment

• Dialysate Conductivity Testing

• Blood Glucose Monitoring

• Administration of Blood and Blood Products

• Infection Control Practices

• Dysrhythmia Recognition and Management
