Responsibilities
- Administer prescribed medications and treatments per physician orders, within scope and facility policy
- Monitor resident status, record vital signs, and report changes to the RN or physician promptly
- Provide routine nursing care and support for activities of daily living; assist with wound care and injections as appropriate
- Document care plans, assessments, and interventions in PointClick Care EMR; maintain accurate and timely records
- Collaborate with the interdisciplinary team to implement and adjust resident care plans
- Float to all units within the same building as required; respond to calls and resident needs to ensure safety and quality of care
- Communicate effectively with residents, families, and team members; provide dementia and memory care support as needed

Requirements
- Current New York Licensed Practical Nurse (LPN) license
- BLS certification
- Minimum 1 year of LPN experience in long-term care or skilled nursing
- Ability to work the evening shift (2:30 p.m.–10:30 p.m.) and every other weekend
- Must be able to float to all units within the building
- Proficiency with PointClick Care EMR or willingness to learn
- NY residency or eligibility for travel assignment within the 75-mile local radius preference
