JOB TITLE: Primary Care Physician
REPORTS TO: Chief Medical Officer/Administrative Manager

About the Role:
As a Primary Care Physician at TRUE HEALTH, you will play a crucial role in providing comprehensive healthcare services to patients across multiple locations in Hicksville, Brooklyn, and Fresh Meadows. Your primary responsibility will be to diagnose and treat a wide range of medical conditions, ensuring that patients receive personalized care tailored to their individual needs. You will also be responsible for developing long-term relationships with patients, promoting preventive care, and managing chronic illnesses. In addition, you will collaborate with a multidisciplinary team to enhance patient outcomes and improve overall health within the community. Ultimately, your work will contribute to the mission of TRUE HEALTH to deliver high-quality, accessible healthcare to all patients.
Minimum Qualifications:
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification in Family Medicine or Internal Medicine.
- Valid medical license to practice in New York State.
- Strong communication and interpersonal skills to effectively interact with patients and staff.
- Commitment to providing high-quality, patient-centered care.
Preferred Qualifications:
- Experience in a primary care setting or community health environment.
- Familiarity with electronic health record (EHR) systems.
- Bilingual proficiency in English and Spanish or other languages.
- Participation in continuing medical education (CME) activities.
- Leadership experience or interest in mentoring junior staff.
- Graduated from an accredited medical school and licensed to practice in New York State.
- Board certified or board eligible status.
- At least 1-2 years of experience preferred.
- Ability to travel to other sites for coverage as needed.
- Bilingual in English and Spanish or Creole/French required.
Responsibilities:
- Conduct thorough physical examinations and medical histories to assess patient health.
- Diagnose and treat acute and chronic illnesses, providing appropriate medical interventions.
- Develop and implement individualized care plans in collaboration with patients and healthcare team members.
- Educate patients on health maintenance, disease prevention, and lifestyle modifications.
- Maintain accurate and up-to-date medical records in compliance with healthcare regulations.
- Provides comprehensive medical care to patients from adolescent years and older in ambulatory care/outpatient settings.
- Provides consultations by examining patients; reviewing medical records, reports, and logs; researching information and developing conclusions and care recommendations.
- Adheres to professional standards, practice policies and procedures, federal, state, and local requirements, and regulatory standards.
- Works as a team player assisting other staff providers with developing diagnosis and patient treatment options.
- Prescribes or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury.
- Orders, performs, and interprets test and analyze records, reports, and examination information to diagnose patients’ condition.
- Obtain and maintain hospital privileges at a Hospital Medical Center in order to provide and coordinate care for patients in the hospital setting.
- Records and maintains patient information such as medical history, reports and examination results.
- Monitors patients’ conditions and progress and re-evaluation treatments as necessary.
- Reviews health risk assessments (HRA’s) to help determine a plan of care.
- Explains procedures and discuss test results or prescribed treatments with patients.
- Refers patients to medical specialists or other practitioners when necessary.
- Participates in LEAN and other activities necessary to implement the multi-disciplinary patient-centered medical home approach for the care of pediatric patients.
- Updates job knowledge by participating in continuing medical educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
- Other duties as assigned.
Skills:
The required skills of clinical assessment and diagnostic reasoning will be utilized daily as you evaluate patients and determine appropriate treatment plans. Strong communication skills are essential for effectively educating patients about their health and fostering a trusting relationship. Interpersonal skills will be critical when collaborating with a diverse healthcare team to ensure comprehensive patient care. Additionally, familiarity with EHR systems will enhance your efficiency in documenting patient interactions and managing health records. Preferred skills, such as bilingual proficiency, will allow you to connect with a broader patient demographic, ensuring that language barriers do not hinder access to care.
SALARY : Commensurate with experience $180,000 - $265,000
