Physical Therapist Job Details

• Location: Eureka, CA 95501 – Northern California healthcare job near redwood forests and Pacific coastline
• Employment Type: Locum tenens (Travel contract)
• Setting: Hospital physical therapy department
• Specialties: Women's Health, Vestibular, Pelvic Rehab, Geriatric Care, Physical Therapy Management
• Shift: Monday to Friday, 8:00 AM – 5:00 PM
• Hours per Week: 40
• Assignment Duration: 13 weeks (Start: 9/29/2025 – End: 12/28/2025)
• Caseload: Up to 6 patients per day (30 productivity points standard)
• Travel Radius: Occasional maximum of 50+ miles; patient visits grouped to minimize drive time
• Estimated Weekly Salary: $2,350 - $2,410

Physical Therapist Qualifications

• Active California Physical Therapist license required
• Minimum 1 year of recent skilled home health or hospital physical therapy experience (within the last 3 years)
• OASIS documentation experience required
• BLS certification required
• Reliable transportation for travel between patient locations in the Eureka, CA area

Physical Therapist Responsibilities

• Deliver high-quality physical therapy services to patients in a hospital setting
• Conduct comprehensive patient assessments and develop individualized care plans
• Implement therapeutic interventions for women's health, vestibular, pelvic rehab, and geriatric care
• Document patient progress and outcomes using OASIS standards
• Collaborate with interdisciplinary healthcare team members to optimize patient care
• Travel to patient locations within the designated radius, ensuring efficient scheduling and clinical support
