Case Manager
Case Manager – Brooklyn (Multiple Locations)
Schedule: Monday–Friday | 9:00 AM – 5:00 PM
Job Type: Full-Time
Work Location: In Person
We are seeking dedicated Case Managers to join our team at multiple locations throughout Brooklyn . This role is ideal for professionals passionate about advocacy, care coordination, and supporting diverse populations.
Key Responsibilities
- Conduct comprehensive assessments to determine client needs
- Develop and implement individualized service plans
- Coordinate and monitor delivery of services and supports
- Advocate for clients and connect them with community resources
- Collaborate with interdisciplinary teams including social workers, healthcare providers, and educators
- Maintain accurate, timely case records and documentation
- Provide crisis intervention and ongoing client support as needed
Requirements
- Associate Degree required
- Bachelor’s Degree preferred
- Strong communication, organization, and advocacy skills
- Ability to work collaboratively in a fast-paced environment
