Job Summary:

We provide excellent rehabilitation services in the comfort of patients’ homes. All treatments are one-on-one, and we strive to help patients return to their community setting.

Homeside Rehab is seeking highly motivated and compassionate Physical Therapists (PT) and Physical Therapist Assistants (PTA) to provide high-quality patient care in the Williamsburg neighborhood of Brooklyn.

Responsibilities:

• Evaluate patients and develop individualized treatment plans based on their needs and goals (PT)

• Implement treatment plans and provide skilled therapy interventions (PTA under supervision)

• Provide evidence-based therapy to improve patients’ function, mobility, and independence

• Maintain accurate and timely documentation of patient care

• Collaborate with the clinical team to ensure coordinated treatment

• Communicate effectively with office staff, patients, and caregivers

Qualifications:

• Active NYS License as a Physical Therapist (PT) or Physical Therapist Assistant (PTA) (Required)

• Preferred: 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings

• Strong clinical and interpersonal skills

Job Details:

• Job Types: Part-time, Per Diem

• Salary: $45.00 – $65.00 per hour (based on role and experience)

• Schedule Flexibility: Choose your own hours; day and evening shifts available, Monday through Friday, with weekend availability as needed
