Outreach Coordinator
Overview
The Outreach Coordinator will connect Damian Family Care Centers, Inc. with its community, developing and working to implement outreach programs that foster relationships to promote the organization’s mission and services.
Essential Job Duties
- Plan, coordinate, and participate in community outreach activities, health fairs, educational workshops, and special events.
- Promote Damian services, programs, and resources to community members and partner organizations.
- Establish and maintain positive working relationships with community agencies, schools, businesses, social service organizations, and healthcare partners.
- Conduct presentations and educational sessions on health-related topics and available healthcare services.
- Assist community members with accessing healthcare services and navigating available resources.
- Support initiatives to increase patient enrollment and retention.
- Assist individuals in scheduling appointments and connecting with appropriate healthcare services.
- Identify barriers to care and collaborate with internal departments to improve patient access.
- Promote preventative care programs, wellness initiatives, and population health efforts.
- Coordinate outreach calendars, community engagement activities, and event logistics.
- Maintain accurate records of outreach activities, participant engagement, referrals, and outcomes.
- Prepare reports and metrics related to outreach efforts and program effectiveness.
- Assist with grant-related outreach activities and data collection when applicable.
- Represent Damian in a professional and positive manner at community events and meetings.
- Collaborate with internal departments including Medical, Dental, Behavioral Health, Registration, and Social Services to support outreach goals.
- Foster partnerships that support Damian's mission and strategic objectives.
- Maintain outreach materials, brochures, promotional items, and educational resources.
- Assist in developing outreach campaigns, communication materials, and social media content as assigned.
- Ensure compliance with organizational policies, HIPAA regulations, and confidentiality standards.
- Participate in staff meetings, training programs, and professional development activities.
Non-Essential Job Duties
- Assist with special projects and community initiatives as assigned.
- Support employee engagement and organizational events when requested.
- Participate in committees, workgroups, and organizational planning activities.
- Provide cross-departmental support during emergency preparedness or community response efforts.
- Perform other duties as assigned to support departmental and organizational goals.
Qualifications & Skills
- High School Diploma/ GED
- Minimum of two (2) years of experience in community outreach, healthcare outreach, patient engagement, public health, marketing, social services, or a related field.
- Experience working with diverse and underserved populations preferred.
- Experience in a healthcare, nonprofit, FQHC, community-based organization, or public health setting preferred.
- Strong interpersonal, communication, and public speaking skills.
- Ability to establish rapport and build relationships with diverse populations.
- Excellent organizational and time-management skills.
- Ability to work independently and as part of a multidisciplinary team.
- Strong problem-solving and critical-thinking abilities.
- Ability to coordinate multiple projects and priorities simultaneously.
- Proficiency in Microsoft Office Suite, including Word, Excel, Outlook, and PowerPoint.
- Ability to collect, analyze, and report outreach data.
- Ability to maintain confidentiality and comply with HIPAA regulations.
- Frequent local travel throughout New York City boroughs and surrounding service areas is required.
- Bilingual English/Spanish preferred.

- Primarily office-based role within a healthcare/clinical setting.
- May require occasional travel between DFCC sites.
- Prolonged periods of sitting, working at a computer, and reviewing detailed financial and operational data.
- May require extended hours during audits, reporting cycles, or system implementations.
- Frequent interaction with staff, leadership, external vendors, and regulatory agencies.
