Job Details Job Job location: Woodland Pond, Inc - New Paltz, NY income Range: Undisclosed DescriptionPosition Summary The Therapy & Rehabilitation Clerk is responsible for day to day operations of the Out Patient Therapy program and Therapy and Rehabilitation Department supplies, and is a coordinator between Nursing and Out-Patient Therapy ESSENTIAL tasks AND RESPONSIBILITIES (*CF indicates Critical Function) Schedules all outpatient therapy treatment sessions, provides schedules to unit clerks, patients, and therapists Involves patient/family in scheduling and accommodates for individual needs (cultural differences, language, etc.) Maintains a positive, professional working rapport with patients, peers and referral sources. Collaborates with therapists, case managers, and insurance companies as needed in regards to schedule, DME needs and completion of necessary documentation Effectively communicates with the interdisciplinary team and the resident regarding the residents therapy program in a timely manner. Willing to assist other members of the interdisciplinary team when asked Provides written and/or verbal communication to referral sources, residents, families and other members of the interdisciplinary team (discharge planning/SW/other therapy personnel/nursing staff) in a timely fashion Assists in performance improvement activities as needed to ensure quality is consistent with established guidelines and to improve the delivery of service rendered Participates in Community educational programs and scheduled meetings as requested by the manager/director Keeps the treatment area clean, organized and safe for patients and staff Communicates with manager/director regarding their schedule and use of nonproductive time to enhance efficiency of both SN and Out Patient departments Provides accurate billing information in a timely manner, assists with compliance with insurance guidelines, and organizes required documentation for reimbursement Completes all required training and in-services All other tasks as assigned QualificationsGENERAL expertise AND ABILITIES Maintains high standards for work areas, attitude and appearance Understands that honesty and ethics are essential in the performance of tasks Willing to lgain and grow Demonstrates maturity in judgment and behaviors Able to work a flexible schedule Maintains predictable and reliable attendance Qualifications High school diploma, or HSE 1-3 years significant working practice preferred Proficiency with computers and standard office software Excellent customer service and verbal communication expertise capability to read/draft instructions, short correspondence, and memos capability to multitask Demonstrated expertise of appropriate expertise for communicating with individuals of all ages, especially the geriatric population
