Job Title: LVN/LPN – Long Term Care (LTC) | Auto Offer

Location: Rochester, NY

Assignment Type: Travel Contract

Duration: 8 Weeks

Schedule: Rotating Shifts | 5x8 Hours

Shift Timing:

• 3:00 PM – 11:30 PM
• 11:00 PM – 7:30 AM
  
  Weekend Requirement: Every Other Weekend

Job Summary

We are seeking an experienced LVN/LPN – Long Term Care (LTC) to provide high-quality nursing care in a skilled nursing facility setting. This role involves supporting patient care, administering medications, and assisting with daily clinical operations while ensuring patient safety and comfort.

Responsibilities

• Provide direct patient care in a long-term care or skilled nursing facility
• Administer medications and treatments as prescribed
• Monitor patient conditions and report changes to the RN or physician
• Assist with care planning and implementation
• Perform basic nursing procedures and support daily living activities
• Maintain accurate documentation in the EMR system
• Supervise and support nursing assistants as needed (charge role responsibilities)
• Ensure compliance with facility policies, safety, and infection control standards

Required Qualifications

• Active New York LPN/LVN License
• Minimum 1 year of LPN experience in a Long-Term Care (LTC) setting
• Previous Charge Nurse experience (required)
• BLS Certification
• Experience in Skilled Nursing Facility (SNF) or Nursing Home
• Epic charting experience (required)

Work Environment

• Skilled nursing or long-term care facility
• Fast-paced setting with a focus on patient-centered care
• Rotating shifts including evenings, nights, and weekends
