Key Job Details – LPN Long Term Care Position
- Location: Rochester, NY 14620 – Prime New York healthcare location near Lake Ontario, parks, and cultural attractions
- Employment Type: Travel LPN Contract
- Healthcare Setting: Long Term Care Facility (LTC)
- Assignment Duration: 17 weeks (September 29, 2025 – January 24, 2026)
- Weekly Hours: 40 hours per week
- Shifts: 8-hour shifts; Days (7am–3pm) and Evenings (3pm–11pm); includes weekends and holidays as needed
- Estimated Weekly Salary: $1250–$1280
- Active Licensed Practical Nurse (LPN) license in New York State
- BLS certification (American Heart Association required)
- Minimum 2 years of recent experience in long term care, skilled nursing, or geriatric nursing
- Strong clinical skills, critical thinking, and ability to work independently
- Dedication to providing patient-centered care with dignity and respect
- Deliver direct patient care to geriatric residents, including medication administration and daily nursing tasks
- Supervise and collaborate with Certified Nursing Assistants (CNAs) and support Registered Nurses (RNs)
- Manage care for 8–10 residents per LPN, ensuring high-quality clinical support
- Provide wound care and monitor resident health (no trach or acute care required)
- Maintain accurate clinical documentation and comply with facility and state healthcare standards
- Promote a positive, respectful, and safe environment for residents and staff
