We have full time availcapability for RN Case Managers. Required availcapability would be Monday through Friday from 9am-5pm and on call hours during Afterhours on weekdays and weekends as needed. Case management work would cover all over Putnam and Westchester counties. The primary workstation will either be our main office in Peekskill or our satellite office at Kendal on Hudson in Sleepy Hollow.
this job offer has a starting income of $92,000 to $95,000 located on practice and currently comes with a $2,000 sign on bonus!

Qualifications required

»Current New York State RN license

»Minimum (1) year recent and verifiable practice with home care agency

»Willing to meet with and speak with patients, their family, and their medical professionals to develop, update and maintain proper care plan

»Strong assessment and organizational expertise

»Prior managery practice with field staff, practice with clinical oversight, monitoring quality assurance, and overall case management

»Demonstrated prosperous track record of working independently

»Ambition to grow and advance beyond current position

»Strong communication and computer expertise
