Peer Specialist

JOB DEFINITION:

The Peer Specialist functions as a member function as a member of the Street Outreach Team to serve individuals experiencing homelessness located in and around Long Island Railroad (LIRR) stations across Nassau County. Advocates for and supports the client, engages with community agencies/health care. Peer Specialist will serve as a role model and/or health coach for clients.

EDUCATIONAL REQUIREMENTS:

High School Diplomas or GED.

EXPERIENCE REQUIRED:

• Relevant life experience required: personal experience living with a disability and/or social/emotional challenge, and willingness to draw upon this experience to serve as a role model to support the recovery model within Health Home model.
• Working knowledge of computer software and electronic health record systems.
• Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards.
• Experienced in and demonstrated comprehensive understanding and working knowledge of the interdisciplinary planning process and the developmental treatment model.
• Familiarity with NYS Entitlements, Housing, Health, and/or Mental Health/Substance Abuse Service Delivery System.
• Knowledge of Medicaid, Social Security and other entitlements preferred.
• Excellent interpersonal skills required.
• Ability and willingness to regularly travel to many locations, sometimes with clients in the vehicle, using various modes of reliable and safe transportation.
• Ability to maintain a clean and valid New York State driver's license.
• Ability to safely operate a vehicle in a safe manner under all driving conditions.

ADDITIONAL QUALIFITCATION:

• Current New York State Driver's License
• Clean DMV Record
• Proof of Motor Vehicle Insurance

DUTIES AND RESPONSIBILITIES:

1. Conduct street outreach to homeless individuals at LIRR stations.
2. Provide face to face outreach to clients to facilitate their engagement in services available.
3. Provide health coaching and recovery support, especially on issues of self-advocacy, wellness self-management, medication adherence, smoking cessation, entitlements, and daily living activities.
4. Assists with and coordinate clients' transportation to appointments, including provision of travel training as needed.
5. Accompany clients to medical and/or other appointments to facilitate their engagement and follow through as needed.
6. Provide concrete service delivery to clients i.e., benefits/enrollments as per program needs.
7. Maintains documents, records, statistics, and other related reports in an organized, timely and accurate manner as per policy and procedure.
8. Acts as a resources/consultant to all team members on psychosocial and/or substance abuse issues and resources.
9. Acts as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services per interdisciplinary care plan.
10. Monitors overall service delivery to clients to ensure coordination and continuity; advocates with service providers/resources as needed.
11. Provides crisis intervention and follow-up.
12. Participate in supervision, training, and education as needed.
13. Must be available weekends and holidays; schedules will stagger between the teams for coverage. Typical shifts will be 7am-3pm and 2pm-10pm.
14. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
15. Complete QI Training during onboarding and participate in QI refresher training as needed.
