Job Type: Contract (Local or Travel)

Duration: 16 Weeks

Shift & Schedule

• Shift: Mid Shift (5x8 Hours)
• Timing: 02:30 PM – 11:45 PM
• Hours per Week: 46.25
• Days per Week: 5
• Weekend Requirement: Every other weekend required
• Orientation: All hours billable

Position Overview

"Navitas Healthcare, LLC" is seeking a dedicated LVN / LPN to join a Long-Term Care (LTC) facility in Vestal, NY. The ideal candidate will have experience working with geriatric patients, including those with Alzheimer’s and dementia, and be comfortable in a fast-paced rehabilitation and nursing environment.

Key Responsibilities

• Administer medications accurately and safely (med pass)
• Monitor patient health, vital signs, and report changes to the care team
• Provide direct patient care, including wound care and daily nursing support
• Assist in developing and implementing individualized care plans
• Deliver compassionate care to patients with Alzheimer’s and dementia
• Maintain accurate and timely documentation in EMR systems (PointClickCare preferred)
• Collaborate with RNs, physicians, and interdisciplinary teams
• Ensure compliance with infection control and safety protocols
• Assist with admissions, discharges, and transfers as needed

Qualifications

• Active New York LPN/LVN License (Required)
• Minimum 1 year of nursing experience (LTC preferred; rehab, med-surg, or hospital experience also considered)
• BLS Certification (Required)
• Experience with medication administration (med pass)
• Strong communication and patient care skills
• Ability to work with geriatric patients, including Alzheimer’s and dementia populations
• Must be able to work the assigned shift without changes after offer
• Local candidates preferred; first-time travelers welcome
