Responsibilities
- Provide direct and indirect skilled nursing care to residents under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and the Interdisciplinary Team.
- Help develop an age-appropriate, culturally relevant care plan and participate in its execution, involving residents, families, and the healthcare team.
- Recommend revisions to the care plan based on resident outcomes and coordinate referrals to address all aspects of care.
- Maintain current nursing practice knowledge, participate in staff mentoring, and uphold safety and quality standards in a geriatric setting.
- Perform duties within the scope of nursing practice, including potential exposure to bloodborne pathogens, and support a collaborative team environment with physicians and ancillary staff.
- Holiday rotations may be required as part of unit coverage.

Requirements
- Active New York Licensed Practical Nurse (LPN) license
- BLS certification (American Heart Association or RQI)
- Minimum 1 year experience as an LPN
- Experience serving in an LPN capacity in a long-term care facility
- Must be able to start full-shift orientation on a Tuesday or Thursday
- Holiday rotations required
