The assignment begins on 07/20/2026 and offers a Monday through Friday schedule with 8-hour shifts.

Job Details:

• Schedule: Monday-Friday, 8:00 am 5:00 pm, minimum of 40 hours per week.
• Required Qualifications: Successful completion of a physical therapy program approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of the American Medical Association; current and unencumbered license to practice physical therapy in New York; 2-3 years of physical therapy experience preferred; current CPR certification; familiarity with computer use including internet, email, and navigating desktop applications.
• Preferred Qualifications: Experience in home health or hospice care is preferred.
• Additional Information: The role requires reliable transportation, a current driver's license, and agency-required liability insurance. The therapist will coordinate services and supervise support personnel in accordance with state-specific regulations. The assignment involves traveling approximately 250-300 miles weekly and utilizing the EMR system PointCare.
