Job Overview:
We are seeking an experienced Registered Nurse (RN) Case Manager for an exciting opportunity in New York. This role is ideal for nurses with strong coordination, communication, and patient advocacy skills who are passionate about ensuring high-quality, cost-effective patient care.

Position Details:

• Unit: Case Management
• Contract Type: Travel / Contract

Key Responsibilities:

• Assess patient needs and develop individualized care plans
• Coordinate care across multidisciplinary teams
• Facilitate discharge planning and transitions of care
• Work with insurance providers for authorizations and utilization review
• Advocate for patients and ensure appropriate level of care
• Maintain accurate documentation and ensure compliance

Requirements:

• Minimum 2+ years of Case Management experience (Required)
• RN experience in acute care or hospital setting preferred
• Strong knowledge of discharge planning and utilization review
• Excellent communication and organizational skills
• Active RN license (state-specific as required)
• BLS certification preferred/required

Preferred Qualifications:

• Experience with EMR systems (EPIC, Cerner, etc.)
• Previous travel experience (for contract roles)
