Job Title: Social Worker – Contract

Location: New York, NY

Contract Duration: 9 Weeks

Schedule: 8-hour shifts | Monday–Friday

Facility Type: Hospital / Healthcare

Weekly Pay: $2,080/week

Position Overview:

We are seeking a dedicated and compassionate Social Worker to join a healthcare team in a hospital setting. The Social Worker will provide support to patients and families by addressing emotional, social, and resource-related needs to enhance overall well-being and facilitate continuity of care.

Key Responsibilities:

• Assess patients’ psychosocial needs and develop individualized care plans.

• Provide counseling and crisis intervention to patients and families.

• Collaborate with interdisciplinary medical teams to coordinate patient care and discharge planning.

• Connect patients and families with appropriate community resources, benefits, and services.

• Maintain accurate documentation within the electronic medical record (EMR) system.

• Advocate for patients’ rights and support emotional well-being throughout hospitalization and recovery.

Requirements:

• License: Active LMSW (Licensed Master Social Worker) required; LCSW preferred.

• Education: Master’s Degree in Social Work from an accredited institution.

• Experience: 6 months–1 year of experience in a medical or healthcare setting preferred.

• Strong communication, interpersonal, and problem-solving skills.

• Ability to manage sensitive situations with professionalism and empathy.

• Proficiency with Microsoft Office and EMR systems.

Preferred Qualifications:

• Experience in medical social work or hospital-based case management .

• Knowledge of local, state, and federal social service programs and community resources.
