Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and other members of the Interdisciplinary Team.
- Contribute to development of an age-appropriate, culturally sensitive care plan, involving residents/families and the healthcare team; adjust plan based on resident outcomes.
- Recommend revisions to the plan of care and coordinate referrals to ensure comprehensive care across all aspects of the resident’s needs.
- Collaborate with residents, families, and team members to maintain a safe, respectful, and compliant care environment; participate in mentoring and orientation of staff as needed.
- Maintain current knowledge and competency in nursing practice; adhere to policies and procedures; apply infection prevention and safety standards.

Requirements
- Current New York Licensed Practical Nurse (LPN) license
- BLS certification (AHA or RQI)
- Minimum 1 year of LPN experience, ideally in a long-term care setting
- Ability to work evenings with rotating D/E/N/W shifts as required
- Demonstrated ability to care for adult and geriatric residents; strong assessment and documentation skills
- Ability to collaborate effectively with RNs, physicians, and other care team members
