Job Title: Registered Nurse Case Manager Profession: Nursing Specialty: Case Management Duration: 13 Shift: Monday - Friday Hours per Shift: 8.0 Experience: - One year of experience as a clinical nurse in a hospital required. - Nine months of experience in home care preferred. License: - Client RN License required. Certifications: - BLS certification required. Must-Have: - Certified Home Health Care experience required. - OASIS experience required. - Telemetry experience required. - EPIC or Netsmart experience preferred. - Strong interviewing, clinical assessment, and communication skills. - Ability to exercise initiative and independent judgment. - Ability to build and maintain professional relationships. - Current driver's license required. - Ability to work independently with minimal supervision. Description: Develops and revises plan of treatments and discharge plans in conjunction with patients, families, physicians, and care delivery teams. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition. Processes orders and notifies physicians of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries. Provides an environment that promotes respect for patients, their privacy, confidentiality, and property. Initiates appropriate preventive and rehabilitative strategies. Consults with and educates patients, families, and other team members regarding disease processes, self-care techniques, and preventive strategies. Provides supervision of LPNs to ensure patient health and safety. Directs, coordinates, evaluates, and supervises the quality of patient care services. Interacts with patients, physicians, referral sources, and others in a manner conducive to positive relationships. Supports Care Management philosophy, promoting quality and cost-effective outcomes. Ensures completion of all clinical records needed for compliance with state and federal legislation. Maintains current knowledge of available community-based services. Demonstrates flexibility and willingness to visit patients based on need. Ensures problem resolution at the point of contact. Attends patient care conferences for patients assigned to care. Monitors results and progress, and takes corrective action on quality indicators. Pursues efforts to reduce or eliminate avoidable costs and errors. Responds to findings of quality management reviews and ensures resolution of potential problem areas. Participates in peer review and quality assessment as assigned. Promotes recognition and respect for the diversity of the workforce. Participates in education, including orientation and in-service training programs. Demonstrates commitment to professional growth and competency. Responsible for key performance indicators established by the manager. Performs other duties as requested. About Cynet Health
