Position Summary:
The Registered Dietitian (RD) is responsible for assessing, planning, implementing, and evaluating the nutritional care of residents in a long-term care facility. The Dietitian ensures compliance with federal and state regulations, including CMS and Department of Health guidelines, and collaborates with the interdisciplinary team to promote optimal nutritional status, quality of life, and clinical outcomes for residents.

Essential Duties and Responsibilities:

  • Conduct comprehensive nutritional assessments for new admissions, quarterly reviews, annual assessments, and significant changes in condition.
  • Develop, implement, and update individualized nutrition care plans in collaboration with the interdisciplinary team.
  • Monitor residents’ nutritional status, including weight trends, lab values, intake records, and skin integrity.
  • Provide recommendations for therapeutic diets, supplements, and tube feeding regimens.
  • Oversee and manage enteral nutrition support, including monitoring tolerance and effectiveness.
  • Participate in care plan meetings and resident/family conferences as needed.
  • Collaborate with the Dietary Manager and food service staff to ensure menus meet regulatory standards and residents’ nutritional needs.
  • Review and approve menus to ensure compliance with CMS, state, and facility guidelines.
  • Monitor and address unintended weight loss, dehydration risk, wounds, and other nutrition-related concerns.
  • Provide nutrition education to residents, families, and staff.
  • Ensure compliance with F-Tags related to nutrition services (e.g., F692, F693, F800, F812).
  • Participate in Quality Assurance and Performance Improvement (QAPI) activities.
  • Maintain accurate and timely documentation in the electronic medical record (EMR).
  • Stay current with regulatory changes and evidence-based nutrition practices.

Qualifications:

  • Registered Dietitian (RD/RDN) credentialed through the Commission on Dietetic Registration (CDR).
  • Minimum of 1–2 years of experience in long-term care, skilled nursing, or geriatrics preferred.
  • Knowledge of CMS regulations and state survey process.
  • Experience with EMR systems.
  • Strong communication and interdisciplinary collaboration skills.
