Social Worker LMSW

Lasante Health Center is seeking a dedicated social worker to provide vital support and advocacy for our community. The ideal candidate will possess a degree in social work, along with strong interpersonal skills.

Responsibilities:

• Adhere to LaSante Health policies and procedures for compliance.

• Ensure timely completion of medical record documentation.

• Obtain Releases-of-Information with patient or legally authorized representative’s permission.

• Conduct individual and family intakes, providing necessary therapy.

• Perform assessments and treatment for assigned cases.

• Contribute to team meetings and clinical rounds.

• Collaborate effectively with assigned clinical head.

• Pursue ongoing professional development and self-care.

• Assisting in additional duties as directed by supervisor

Qualifications:

• Education: Social Work Degree

• License/Certification: Licensure as an LMSW in New York State

• Strong interpersonal skills
