Case manager
The Housing Works Emergency & Transitional Housing Program (ETH) provides time limited transitional housing and support services to individuals reentering the community following periods of incarceration or other justice system involvement. Program participants engage in a variety of onsite services, including case management, vocational training, housing advocacy and mental health support that address barriers to accessing housing in the community and help build skills that participants can use to achieve and maintain stable functioning in the community. The Housing Case Manager provides direct case management and housing placement assistance services to participants enrolled in the ETH Program, including, but not limited to, conducting psychosocial assessments, co-developing and reviewing housing and treatment plans, connect individuals to community-based services to address social determinants of health, benefits and entitlements enrollment and assisting participants with identifying and preparing for more permanent housing. This is an ideal opportunity for reentry or housing services professionals with case management or housing placement experience looking to work alongside like-minded professionals committed to improving outcomes for justice involved individuals.
Responsibilities
- Manage a caseload of approximately 35–45 clients enrolled in the ETH Program.
- Conduct psychosocial assessments with all newly enrolled clients.
- Conduct housing eligibility screenings to inform and develop housing plans of care.
- Co-develop robust Treatment Plans and Housing Plans; review every 30 days.
- Meet with clients regularly per program workflow and contract deliverables.
- Assist clients in obtaining vital documents (e.g., government-issued ID, Social Security card, birth certificate).
- Support clients with benefits and entitlements enrollment.
- Help clients with housing applications and schedule apartment viewings.
- Assist with family reunification efforts.
- Connect clients to internal and external health, wellness, and support services.
- Escort clients to service appointments and coordinate alternative transportation as needed.
- Engage in care coordination with internal and external providers.
- Conduct case conferences with clients’ interdisciplinary care teams.
- Provide crisis intervention when necessary.
- Maintain accurate and up-to-date client charts using Housing Works EMR systems.
- Adhere to program and agency quality assurance standards.
- Participate in quality improvement activities.
- Submit daily, weekly, monthly, and ad hoc reports on time.
- Attend program and agency meetings.
- Promote Housing Works’ core values in daily activities.
- Perform other tasks as assigned by supervisor.
Secondary Responsibilities
- Provide culturally competent services per Housing Works policies and practices.
- Engage in advocacy efforts as outlined by Housing Works and the Justice Initiatives Department.
- Work effectively with clients of diverse backgrounds and varying service needs.
- Collaborate with team members within the Justice Initiatives Department and across the agency.
Other Functions
- Attend and participate in staff meetings and scheduled trainings.
- Respond promptly to emails, texts, and phone calls.
- Flexibility to work pre-scheduled evening hours to meet client service deliverables.
Education/Skills Requirements
- Bachelor’s Degree in social services, psychology, social work, or related field.
- Case management or direct client service experience.
- Experience with individuals involved in the criminal legal system.
- Experience with individuals with substance use or mental health diagnoses.
- Strong written documentation and communication skills.
- Good problem-solving skills and ability to work independently.
- Proficiency in Microsoft Office Suite, especially Excel.
- Experience using Electronic Medical Records (EMR) systems.
- Preferred Competencies :
- Bilingual in Spanish (preferred but not required).
- Excellent oral and written communication.
- Strong technical skills for Microsoft Office, email, and case management systems.
- Ability to work both independently and collaboratively.
- Dependability and punctuality for scheduled shifts.
