Program manager
Position Summary
Manage day-to-day operations of 6 Case workers, and 2 Patient Tenant Coordinators. Working under the guidance of the Regional Director of Operations, the Program Manager will be responsible for the operations of the program and its staff based on the Social Delivery Plan. Responsible and accountable for ensuring achievement and excellence in the administration and delivery of program services. The One Stop Program Manager is responsible for thoroughly understanding R.A.I.N. policies and procedures, DIFTA Performance Standards and other pertinent regulatory standards and contractual requirements. The One Stop Program Manager is responsible for ensuring compliance and program utilization levels accordingly. This position requires a leader with a strong program management and administrative background and knowledge. Lastly, the One Stop Program Manager has oversight over four revitalization sites, including budgeting, reporting, staffing and operations. Essential Duties And Responsibilities- Manage day-to-day operations of 6 Case workers, and 2 Patient Tenant Coordinators
- Conduct potential client intake procedures to determine initial eligibility onto Peer place.
- Conduct recurring and event-based in-home assessments to determine client needs & concerns, so as to evaluate initial & ongoing program eligibility.
- Assess and regularly track changes to the clients cognitive & functional limitations, as well as family circumstances, financial resources, informal/formal support status, home environment, and any additional factors linked to the clients safety and ability to carry out activities of daily living.
- Provide accurate and appropriate information on services, benefits, and entitlements that address the clients needs.
- Arrange for provision of appropriate services for the client (as established in the clients care plan) and advocate/intervene with other agencies on the clients behalf.
- Provide crisis intervention and goal-oriented counseling to client and/or caregiver(s).
- Conduct follow-up and monitoring to ensure clients needs are being met.
- Maintain complete, detailed and accurate case records.
- Input client and service information in DFTAs client tracking system.
- Attend all required DFTA trainings.
- Comply with DFTA regulations and Case Management Standards of Operation.
- Represent the agency in the community and in interagency & intracompany activities.
- Complete duties assigned by Supervisors, the Program Director, and Assistant Director in a timely and effective manner.
- Perform all other duties as assigned by the Supervisors, Assistant Director, Program Director, Executive Director and CEO
- Masters degree in Social Work (MSW), Gerontology, or other Human Services field with at least one year of supervisory or administrative experience, preferably with the elderly,
