Director of Community Outreach and Navigator with Choose Healthy Life

Position Overview

The Abyssinian Baptist Church is seeking a dynamic, mission-driven individual to serve as the  Director of Community Outreach and Health Navigator  for the  Choose Healthy Life (CHL) initiative. This is a dual-role position that combines  strategic oversight  of community outreach with  hands-on leadership  of the CHL public health programming.

Working closely with the church's senior leadership and  United Way of New York City (UWNYC) , the Director/Navigator will develop and lead outreach strategies, foster community partnerships, and execute targeted health initiatives to improve wellness in Harlem's Black communities—especially among NYCHA residents and other underserved populations.

Key Responsibilities

As Director of Community Outreach

  • Lead the church's overall outreach vision and strategy, aligning with CHL's mission and community priorities.
  • Develop partnerships with local organizations, healthcare providers, and public agencies to expand the church's public health impact.
  • Build and maintain a comprehensive outreach calendar, integrating CHL events and broader community wellness efforts.
  • Coordinate with clergy, ministry leaders, and church staff to ensure cross-functional collaboration and engagement.
  • Represent the church at public forums, community boards, and interfaith coalitions to advocate for health equity.

As CHL Health Navigator

  • Design and implement a CHL Action Plan based on community health assessments and church priorities.
  • Plan and host CHL wellness events such as  Black Family Wellness (BFW) Days , screenings, and educational workshops.
  • Conduct targeted outreach and direct engagement, prioritizing NYCHA communities and faith-based networks.
  • Track community participation, submit weekly program reports, and input engagement activities into CHL Event Tracker.
  • Attend regular CHL trainings and participate in national bi-weekly calls with Health Navigators.
  • Promote health literacy and increase access to preventive services through personalized navigation and community education.

Qualifications

  • Education:
    • Associate's Degree required; Bachelor's Degree strongly preferred.
  • Experience:
    • 2–5 years in public health, community engagement, or nonprofit program management.
    • Demonstrated success in faith-based or culturally rooted community initiatives.
  • Skills:
    • Strong leadership, communication, and organizational skills.
    • Comfortable managing both strategic planning and hands-on implementation.
    • Experience collecting, reporting, and analyzing outreach and program data.

Preferred Skills & Attributes

  • Deep understanding of issues affecting Black and BIPOC communities.
  • Proficient in digital tools such as Connect.UWNYC and FindHelp platforms.
  • Culturally competent with strong interpersonal and community-building skills.
  • Bilingual abilities a plus (not required).
  • Passionate about health equity, faith-based service, and social justice.

Work Schedule & Expectations

  • Full-time role; must be available on select weekends and evenings to support church and community events.
  • Reports to the church's senior leadership and collaborates regularly with UWNYC.
