@Home Therapy is seeking an Occupational Therapist to provide in-home services to adult patients in the Jericho area. This role focuses on improving safety, preventing functional decline, and helping individuals maintain independence in their daily routines.

Working in patients’ homes allows you to deliver highly relevant, practical care that directly translates into better day-to-day outcomes.

Key Responsibilities:

Home Safety & Risk Assessment:

• Evaluate living environments for fall risks and accessibility challenges.
• Recommend modifications and adaptive equipment to improve safety.

Functional Independence Training:

• Assist patients with essential daily activities such as bathing, dressing, and household tasks.
• Introduce techniques to conserve energy and reduce strain.

Personalized Care Planning:

• Develop individualized therapy programs based on each patient’s condition and home setup.
• Adjust interventions to reflect progress and changing needs.

Caregiver Support & Education:

• Instruct family members and aides on safe assistance techniques.
• Promote consistency between therapy visits.

Clinical Documentation:

• Maintain accurate and timely records using an electronic documentation system.
• Ensure compliance with all regulatory standards.

Qualifications:

• Active New York State Occupational Therapist license.
• Experience with adult or geriatric populations preferred.
