Position: Nurse Practitioner – Nursing Home or House Call

Position Summary The facility is seeking a dedicated and compassionate Nurse Practitioner (NP) to join its expanding Nursing Home and House Call program across the NYC boroughs . This opportunity offers flexible scheduling (Full-Time or Part-Time, including weekends only) and allows providers to build meaningful, long-term relationships with patients in a low-stress, continuity-focused environment. You’ll work independently, supported by a collaborative team of physicians, nurses, and care coordinators, to deliver quality, evidence-based medical care directly in the patient’s living environment. Essential Position Responsibilities- Conduct comprehensive patient evaluations and follow-up visits in nursing homes, assisted living facilities, or patients’ homes.
- Manage acute and chronic medical conditions with a focus on preventive and geriatric care.
- Order, interpret, and follow up on diagnostic tests and lab results.
- Coordinate care with physicians, specialists, and interdisciplinary care teams.
- Provide patient and family education regarding treatment plans and health maintenance.
- Document visits and treatment plans accurately in the electronic health record (EHR).
- Participate in care transitions and collaborate with facility staff to ensure continuity and quality of care.
- Education: Graduate of an accredited Nurse Practitioner program (MSN or DNP).
- Licensure & Certification: Active or eligible Nurse Practitioner license in New York; national certification required.
- Experience: Prior experience in primary care, geriatric medicine, home care, or nursing home settings preferred, but new graduates are welcome to apply.
- Skills: Strong communication, clinical judgment, and time-management skills with a compassionate, patient-first approach.
