About the Position
Shift
- Day shift
- 8 working hours per shift
- Contract guarantee: 40 hours/week
Responsibilities
- Provide skilled home healthcare visits to patients in their homes
- Perform wound care (including wound vac management)
- Manage and maintain PleurX drains, PICC lines, nephrostomy tubes, and ostomy care
- Perform dressing changes and sterile procedures
- Conduct blood draws and venipuncture
- Support diabetes management care plans
- Manage CHF (Congestive Heart Failure) patients
- Deliver independent nursing care across varied home environments
- Travel to patient homes (approx. 50–75 miles/day) and document care accurately
- Coordinate care as needed while working independently in the field
Requirements
Requirements
- Active New York State RN License
- BLS (Basic Life Support) certification
- 1–3+ years of home care experience
- At least 1 year recent clinical nursing experience
