Social Worker
SOCIAL WORKER – Inpatient, Full Time
Immediate position available at Park Terrace Care Center, in Queens, NY in our certified sub-acute brain injury rehabilitation program.
Work as part of a highly skilled interdisciplinary team in a fast paced, fun and exciting environment!
Residents admitted into this unique, intensive program are adults aged 16 and over with a range of acquired brain injuries such as TBI, stroke, brain tumors, encephalitis, and anoxic brain damage. Length of stay is typically approximately 3 months to a year, and the majority of residents return to living in the community upon completion of their rehabilitation program. Specific responsibilities include a variety of psychosocial services such as:
- serving as a liaison between the care team, residents and their families
- providing emotional support to the residents and their families
- participating in formal/informal interdisciplinary meetings
- interfacing with and maintaining relationships with staff and administration
- completing documentation of assessment and progress notes in a timely manner
- developing individualized care plans and maintaining accurate records
- coordinating discharge planning and continuum of care for the residents and their families
Candidate must be a LCSW/LMSW/LMHC eligible, or licensed in related field. Hospital or long-term care experience preferred. Experience working with brain injury highly desirable, but not required.
