We are seeking an experienced and compassionate Nurse Practitioner (NP) to join our team providing care in Skilled Nursing Facilities (SNFs) . This full-time role offers stability, strong clinical autonomy, and the opportunity to provide meaningful care to patients in a post-acute and long-term care setting.

This position is ideal for a Nurse Practitioner who enjoys managing complex patient populations, working collaboratively with interdisciplinary teams, and practicing in a structured facility environment.

Responsibilities

• Perform comprehensive patient assessments in Skilled Nursing Facility settings
• Diagnose and manage acute and chronic medical conditions
• Conduct routine patient rounds and follow-up visits
• Collaborate with physicians, nursing staff, and interdisciplinary team members
• Order and interpret labs, diagnostic studies, and treatment plans
• Educate patients and families regarding care plans and treatment goals
• Complete accurate and timely documentation in the EMR
• Ensure compliance with Medicare and facility regulations

Qualifications

• Active Nurse Practitioner (NP) license
• Experience in Skilled Nursing Facilities, post-acute care, or long-term care required
• Strong clinical decision-making and patient management skills
• Comfortable managing a patient panel independently
• Ability to work collaboratively with physicians and facility staff
