Ten Broeck Center for Rehabilitation & Nursing is now hiring a full time Social Worker to join our team! Ten Broeck is a 258-bed facility that is recognized all over the Hudson Valley for our outstanding delivery of subacute and long-term care.
The Social Worker is responsible for coordinating activities with the community, outside facilities and agencies that can provide economic, psychological and medical assistance. They help the resident, staff and families with issues that impact or prevents adjustment or any other aspect of the resident's quality of life.The social worker consults with the Director of Resident and Family Services and works as part of an interdisciplinary team to enhance the social and emotional well being of the resident. ESSENTIAL FUNCTIONS OF A SOCIAL WORKER:
·Regularly assesses the social and emotional needs of the resident, sets appropriate goals for meeting those needs within the overall plan of care, and provides continuing and regular services to meet the needs and achieve goals.
·Reports all cases of alleged violations involving mistreatment, neglect or abuse, including injuries of unknown sources.
·Attends meetings as assigned.
·Responsible for providing services to families where problems or attitudes may interfere with the resident's medical care.
·Responds to referrals initiated by other departments in a timely manner.
·Assists residents and responsible party on day of admission with appropriate forms.Assures the resident's civil, personal and property rights are protected and that both the resident and responsible party understands these rights.Explains all forms of regarding the Self-Determination Act on admission.
·Completes all documentation timely.Responsible for manual as well as computerized documentation.
·Completes screens according to New York State Mandates.
·Completes Social Service section of the Minimum Data Set forms as assigned.
·Participates in the interdisciplinary care plan meetings and invites the resident and family to attend.Reports on assigned residents, covering Resident Assessment Protocol and Triggers.
·Prepares resident and family for transfer or discharge of resident (room transfers, transfer or discharge to another facility or to the community).Provides appropriate services and preparation for orderly and appropriate discharge or transfer, and documents in the medical record.
·For resident hospitalization, explains bed retention policy to resident and/or responsible party verbally and in writing.Documents notification in medical chart.
·Provides consultation to other departments regarding individual resident or general situations that may affect the social and emotional well being of residents.
·Personal Trust Funds.Social Worker will contact families if funds not available to purchase needed items.
·Acts as a discharge planner.
·Assists with burial arrangements if no family is available.
·Holds therapeutic groups, to include appropriate inservices to staff as needed.
·Ensures that information regarding residents is held confidential.
·Attend department meetings.
·Assist with transportation, if needed, to and from activities.
·Assist resident to participate in individual activities of choice (i.e.: set up of equipment, positioning assistance, providing sufficient lighting and space).
·Inform residents of various activities available to them even when Activities staff are not available.
·Answers call bells in a polite and timely manner.
·Assures safety of the residents at all times.



PHYSICAL REQUIREMENTS OF A SOCIAL WORKER:

»Push a wheelchair.

»Lift a medical chart.

»Be able to visit each assigned resident within his or her domain.
