- Shift: Days (40 hours/week) with rotating weekend coverage
- Salary Range: $58240.00– $77168.00 per year
- Bonus: $7,500 Sign on Bonus
Requirements
- Bachelor of Science degree in Human Nutrition and Dietetics (required).
- Registered Dietitian (RD) or Registration Eligible (required).
- Certified Dietitian/Nutritionist (CDN) in New York State (preferred).
- 1 year of acute care or long-term care experience (preferred).
Responsibilities
- Conduct nutrition assessments and identify patients/residents at risk for malnutrition.
- Perform Nutrition Focused Physical Exams (NFPE) to assess malnutrition levels.
- Develop individualized diet plans and monitor progress/effectiveness of interventions.
- Provide nutrition education and counseling to patients/residents, families, and staff.
- Collaborate with physicians, nurses, and care teams to integrate nutrition into overall care plans.
- Maintain accurate documentation in compliance with facility and regulatory standards.
