JOB PURPOSE:
Responsible for the implementation and coordination of the Services to Older
Refugees Program (SORP), which provides comprehensive case management, outreach,
naturalization assistance, and community engagement activities for older refugees and eligible
immigrant populations. The SORP Coordinator will provide culturally and linguistically
appropriate services that support healthy aging, independent living, access to mainstream aging
services, and social integration for older refugees in the Mohawk Valley region. Preference will
be given to bilingual candidates with the ability to effectively communicate with refugee and
immigrant communities served by The Center.

DUTIES: Coordinate and implement all program activities related to the Services to Older
Refugees Program, including outreach, intake and assessment, case management, referrals,
community education workshops, socialization and engagement activities, interpretation and
translation coordination, and naturalization assistance for eligible older refugees. The SORP
Coordinator will work collaboratively with internal departments and community partners to
ensure older refugees have access to culturally responsive services that promote wellness,
independence, citizenship attainment, and community integration.

QUALIFICATIONS:
• Associate’s degree in human services, social work, public health, gerontology, or related
field preferred; equivalent combination of education and experience considered.
• Experience working with refugee, immigrant, and culturally diverse populations required.
• Knowledge of aging services, public benefits systems, and community-based resources
preferred.
• Demonstrated experience with case management, community outreach, and program
coordination.
• Strong verbal and written communication skills and presentation skills.
• Computer proficiency including Microsoft Office Suite and experience with data entry
and database management.
• Ability to maintain accurate documentation and confidential records.
• Strong organizational, interpersonal, and problem-solving skills essential.
• Ability to work collaboratively with community partners and multidisciplinary teams.
PRIMARY FUNCTIONS:
• Conduct outreach and engagement activities to identify and recruit eligible older refugees and immigrants for participation in the SORP program.
• Complete participant intake, determine eligibility, conduct comprehensive assessments of needs, and develop individualized case management plans.
• Provide ongoing case management services and regularly update participant service plans to address changing needs.
• Coordinate referrals to community agencies and mainstream aging service providers including the Oneida County Office for the Aging, 50 Forward Mohawk Valley, and other partner organizations.
• Organize and facilitate community education workshops and culturally responsive educational activities for older refugees and community stakeholders.
• Coordinate and implement socialization and community engagement activities that reduce isolation and increase opportunities for participation in community life.
• Provide or coordinate interpretation and translation services to ensure meaningful access to services and information.
• Assist eligible participants with naturalization and citizenship services, including naturalization counseling, citizenship referrals, assistance with forms and waivers, and coordination of English/Civics instruction.
• Maintain participant files, case notes, service documentation, and program data in compliance with agency, state, and federal requirements.
• Track program outcomes and prepare reports required by funding agencies.
• Participate in cross-training activities with community organizations and service providers to strengthen community capacity to serve older refugees.
• Promote the SORP program through presentations, workshops, coalition participation, and community events.
• Collaborate with other agency departments to ensure comprehensive service delivery for clients.

OTHER FUNCTIONS:

• Ensure program activities are administered in accordance with federal and state regulations and in compliance with contract and funding requirements.
• Maintain collaborative working relationships with community agencies, aging service providers, healthcare providers, and refugee-serving organizations.
• Participate in community coalitions, meetings, and trainings related to aging, refugee services, and immigrant integration.
• Assist with quality assurance activities, monitoring, and evaluation efforts related to program performance.
• Any other duties required by the agency, contracts, supervisory management, or Board of Directors.
