Home Care Assessment Nurse - RN - Registered Nurse

We are currently seeking a dedicated Home Care Assessment Nurse (RN) to join our team and conduct comprehensive assessments for patients in their home environments.

As a Home Care Assessment Nurse, you will play a critical role in evaluating patient needs, developing care plans, and ensuring the highest standards of care in home settings. Pay Rate: $80 per assessment

Pay Period: Sunday to Saturday

Pay Schedule: Weekly, Every Thursday via Direct Deposit

Responsibilities

• Conduct initial and ongoing assessments of patients in their home environments to determine healthcare needs and appropriateness of home care services.
• Develop individualized care plans based on assessment findings and collaborate with interdisciplinary teams to implement care.
• Educate patients and families about care plans, promoting understanding and adherence to treatment regimens.
• Document all assessments, interventions, and patient responses accurately and timely in electronic health records.
• Evaluate the effectiveness of care plans regularly and adjust them as needed to meet patient needs effectively.
• Act as an advocate for patients, ensuring their rights, preferences, and healthcare needs are respected.
• Stay current with best practices in home care nursing and participate in training and professional development opportunities.

Requirements

• Current and valid state licensure as a Registered Nurse (RN).
• Minimum of 2 years of nursing experience, preferably in home health or community settings.
• Strong critical thinking, assessment, and clinical judgment skills.
• Excellent communication skills, with the ability to educate and build rapport with patients and families.
• Knowledge of home care regulations and standards of practice.
• Ability to work independently and manage time effectively in various home settings.
