Position Overview

The Care Navigator plays a vital role in connecting individuals to the healthcare and social support services they need. This role focuses on outreach to Medicaid members , helping them understand, access, and enroll in available benefits and programs - with a strong emphasis on phone-based member engagement .

Key Responsibilities

• Conduct high-volume outbound calls to Medicaid members.

• Initiate conversations about benefits and complete an initial health and living situation assessment .

• Explain programs and services available through the 1115 Waiver .

• Assess members' eligibility for Health-Related Social Needs (HRSN) services (housing, food, transportation, etc.).

• Connect members to appropriate local, state, or federal resources .

• Document all outreach, contact attempts, and member consent according to program standards.

• Communicate with internal teams and community partners to ensure referrals are completed or redirected as needed.

• Maintain accurate, detailed progress notes and records.

• Collaborate with supervisors and team members to meet outreach goals.

Qualifications

• Strong communication and customer service skills; able to build rapport and trust over the phone.

• Prior experience with Medicaid populations or public health programs preferred.

• Bilingual (Spanish/English) highly preferred.

• Detail-oriented and organized, with the ability to manage a fast-paced outreach workload.

• Comfortable making outbound calls and speaking with members regularly.

Preferred Experience

• Familiarity with the following New York regions: Finger Lakes, Hudson Valley, Bronx, Capital District .

• Knowledge of NYS Medicaid and/or the 1115 Waiver .

• Experience in Case or Care Management .

• Prior work in a start-up or evolving environment .

Ideal Candidate Attributes

• Knowledge of case management concepts (though not performing direct care management).

• Comfortable working independently and taking initiative.

• Proactive and not afraid to make mistakes, learn, and problem-solve.

• Strong critical thinker who can provide examples of workplace challenges solved.

• Understands this role is not part of a Community-Based Organization and does not involve ongoing case management with members.
